Q: Do you accept my insurance?

Q. I have pain when I move, but have not seen my doctor. Can I still make an appointment?

A: YES! We can make an appointment for a free 15 minute consultation. After that, If physical therapy is needed, we will discuss your situation and coordinate what is needed from you, your doctor, and your insurance.

Q. I have doctor's orders for physical therapy. Is Performance Lifestyles in my doctor's network?

A. Your health insurance allows you to go to an in-network PT provider. As long as we accept your health insurance, we are considered in-network, and you can be treated by Performance Lifestyles.

Q. What can I expect on my first visit?

A. Bring with you the doctor's prescription (if you have one), your insurance card, and wear loose fitting clothing. You will fill out a patient information form, a health history, and a consent. We will make a photocopy of your insurance card. Once these are completed, your evaluation will last about an hour and will involve talking about your physical condition, then assessing and taking measurements such as range of motion and strength, and as time allows, we will begin treatment. We will discuss your treatment plan, and schedule your follow up appointments. You can download the (3) intake forms below to fill out prior to your appointment.
