Q: Do you accept my insurance?

A: We accept most major health insurances, workman's compensation, auto insurance, and private payment arrangements. Call us for confirmation and approval.

Q. I have pain when I move, but have not seen my doctor. Can I still make an appointment?

A: We can make an appointment for a free 15 minute consultation. After that, If physical therapy is needed, we will discuss your situation and coordinate what is needed.

Q. I have doctor's orders. Is Performance Lifestyles in my doctor's network?

A. Your health insurance allows you to go to an in-network PT provider. As we accept most major health insurances, we are certainly in their network, and you can be treated by Performance Lifestyles.

Q. What can I expect on my first visit?

A. Bring with you the doctor's prescription, your insurance card, and wear loose fitting clothing. You will fill out a patient information form, a health history, and a consent. We will make a photocopy of your insurance card. Once these are completed, your evaluation will last about an hour and will involve talking about your physical condition, taking range of motion and strength measurements, and as time allows, beginning treatment. We will discuss the treatment plan, and schedule your appointments.
